Your sleep diary: what to write down
A sleep diary should describe your usual pattern and daytime effects without requiring perfect estimates.

Use a repeatable format
Record approximate bedtime, wake time, awakenings, and how you felt during the day. Include relevant changes in routine when useful. Mark estimates as estimates instead of watching the clock to obtain precise numbers.
Use it to support a conversation
Summarize the pattern and the effect on work, study, or ordinary activities. Ask a professional which details to continue and for how long. A diary organizes information; it cannot establish a sleep diagnosis by itself.
Practical checklist
- Record bedtime, waking time and how rested you feel.
- Add caffeine timing, naps and anything that disturbed the night.
- Bring the diary to an appointment and describe daytime effects such as trouble concentrating.
In the Philippines, contact your usual health center, pharmacist or appropriate clinician for individual advice. Service availability and fees vary; ask the provider directly.
